ElderGrade is a website. We cannot inspect a facility, remove a resident or compel anyone to do anything. These three can, and all of them are free.

  1. 1. Someone is in immediate danger

    Call 911, and then Adult Protective Services in your state. Do not wait on any process, including this form.

  2. 2. Your state's Long-Term Care Ombudsman

    Free, confidential and independent of both the facility and the state. Advocating for residents is their entire job, and they can attend meetings with you.

  3. 3. File a complaint with your state survey agency

    This is the one that triggers an investigation. Complaint investigations produce a large share of the most serious citations in the federal record — including many that a routine annual survey would never have found.

Tell us too

Optional, and never published. We use reports to spot patterns — when several families describe the same problem at the same home, that is worth investigating even when the federal record is silent.

Please do not include a resident's medical records or anything you would not want stored in an ordinary inbox. This form is not monitored around the clock and is not an emergency service.
