Every number on this site comes from the CMS Provider Data Catalog — the same public files behind Medicare's own Care Compare. Nothing here is surveyed, submitted by facilities, or estimated.

Check it yourself

All of these files are free and public at data.cms.gov/provider-data. Individual facility records can be cross-checked at Medicare Care Compare.

What we add

We do not alter the federal figures. What we compute on top of them is: the ElderGrade composite score and letter, the weekend staffing gap, staffing measured against case-mix expectation, per-state and per-county ranks, and citation counts rolled up by severity. All of it is derived arithmetic on published fields, and the formula is on How we grade.

Limits worth knowing

  • CMS republishes on a cycle. A home that changed hands or leadership recently may not yet look different here.
  • The quality-measure component is built from assessments completed by the facility itself.
  • The inspection star rating is scored relative to other homes in the same state, so cross-state comparison of that one component is imperfect.
  • Private-pay rates are not in any federal file. We publish none rather than estimating.
  • We hold no photographs of these facilities, so profiles carry none. Editorial photography elsewhere on the site is openly licensed and credited.

Corrections

If a facility believes its federal record is wrong, the correction has to be made with CMS and the state survey agency — we publish their file and cannot amend it on request. If we have made a processing error, tell us and we will fix it.
